Products contained on the PMB main list will not be reimbursed from the acute medication benefit, after one claim from acute benefits, for one of the following reasons:

1. Specific clinical criteria are required to be met before funding will be approved from the chronic medication benefit. Applications for this medication should be submitted with the specified clinical information (as indicated) to the Medicine Risk Management department for pre-authorisation.
2. Pre-authorisation required* indicates that the product may be considered for reimbursement from the chronic medication benefit upon motivation, if the requested diagnosis is covered within the Fund's schedule of benefits.
