What is a treatment plan and how does it work?
A treatment plan letter is effectively a schedule of services specific to the CDL condition(s), and provides benefits for doctor/specialist consultations, diagnostic tests e.g. pathology, radiology etc., as well as other fundamental services in managing your chronic condition(s) effectively.
We encourage you to share this plan with your treating doctor to ensure that the care provided to you is according to the treatment plan. Additional services will be considered based on supporting clinical motivation, should you require additional medical attention. Members who fall under this category will be assigned a wellness coach to further assist them in managing their health.

How to ensure that your claims are paid
To ensure that the services indicated on the treatment plan are paid from the correct benefit, your provider needs to submit the claim with the correct and relevant ICD 10 codes, as they appear on your treatment plan letter.
